Sports Physicals
Get Ready for the Season
Sports physicals help ensure that student athletes are ready to safely participate in their chosen sport. Our providers perform a thorough physical examination to assess overall health and identify any concerns that may need to be addressed before participation.
Sports physicals are available for students participating in school and recreational sports.
What to Expect
During a sports physical, the provider will review relevant health history and perform an examination that may include:
- Height and weight
- Blood pressure and pulse
- Vision screening
- Heart and lung examination
- Musculoskeletal examination
- Evaluation of joints and range of motion
- Review of previous injuries and medical history
- Assessment for any concerns that may affect sports participation
Sports Physical Forms
No need to track down the right sports physical form before your appointment. We provide all necessary paperwork at our office.
To help keep your appointment running smoothly, we encourage patients to arrive 10–15 minutes early to allow time to complete the required paperwork before the physical.

DOT Physicals – coming soon!!
Commercial Driver Medical Examinations
If you drive commercially, you may be required to complete a Department of Transportation (DOT) physical examination to determine whether you meet the federal medical requirements for commercial driving.
Our clinic provides DOT physical examinations for commercial drivers and can help make the process as straightforward as possible.
What to Bring to Your DOT Physical
To help your appointment go smoothly, please bring:
- Your driver’s license or other photo identification
- Most recent DOT certificate
- A list of current medications
- Information about your medical history
- Glasses or contact lenses, if applicable
- Hearing aids, if applicable
- Any relevant medical documentation requested by your provider
If you have a medical condition or take prescription medications, bringing documentation from your treating provider may help prevent delays in completing your examination.
During Your DOT Physical
The medical examiner will review your medical history and perform the required examination and screenings. Depending on your individual circumstances, additional documentation or testing may be necessary before your certification can be completed.
Please contact our office if you have questions about what to bring or what to expect during your DOT physical.
